This document is a Privacy Notice for the patients of Harley Street Ambulance Service, London Group Business Park, 715 North Circular Rd, London, NW2 7AQ; and is effective from 21st June 2018.

Privacy Notice for Patients
1) Personal Data We Collect
When an appropriate referrer or an appropriate relative contacts us, we may collect information about you, including your name, physical address, telephone number, physical and mental health, credit card details (when relevant).
If the service we provide for you is commissioned by third parties (e.g. hospitals, etc) they will provide us with a variety of information, including your name, contact details, date of birth, medical history, mental health and physical health history.
When we transport you to hospital, or when we transport you home, we will keep information about you in your personal records like your name, date of birth and details of any care and / or treatment that you are having or have had in the past. We will regularly update the information about your ongoing care and / or treatment.
2) Collecting and Using Personal Data
You may be asked to provide your personal information when you are in contact with Harley Street Ambulance Service. We will always use your information in accordance with this Privacy Notice.
We may also combine it with other information to provide and improve our services.
If you choose not to provide us with the information we request, in most instances, we will not be able to provide you with our care provision or respond to any queries you may have.

The personal information we collect is stored primarily in computer format. Irrespective of the mode of storage, we have appropriate and adequate admin and IT processes in place, to make sure that all of your information is kept safe.

The personal information we collect and store about you allows us to provide care and services to you. We also use it to help us develop, operate and improve the quality of the care we provide and the type of services that we offer.
As and when required, we may use your information to send important notices and updates to you or to those acting on your behalf (e.g. updates to your care and/or treatment plans). The sharing of this information is important to your interaction with Harley Street Ambulance Service, and you may not opt out of receiving these communications.
We may also use personal information to assist us in conducting audits and quality assurance, so that we can improve our services.
We may use your personal information to test our computer systems such as the software we use to store your health, social and care records to improve our services. If you do not wish for your information to be used for these purposes, you can let us know.
We don’t sell, share or give information to third parties for marketing purposes.
5) Keeping Your Personal Information Safe
Harley Street Ambulance Service takes the security of your personal information very seriously. So as to ensure that your personal information is kept safe, we have a series of admin and IT measures in place. Access is limited only to those employees who need to access it to provide you with the necessary care and support.
It is mandatory that all staff must undertake annual Data Security and Data Protection training.
Our privacy and security guidelines and our expectations in relation to Data Security are communicated to all Harley Street Ambulance Service employees. These privacy safeguards are strictly enforced.
We send any information that we need to share with any third parties either as a legal requirement or through an anonymised process.
All data is stored on servers within data centres in the UK.
6) Disclosing Personal Information to Third Parties
We will only share your personal information with third parties as follows:
•Where you have given your consent to the information being shared;
•Where there are concerns about your or others health and safety; or
•Where there is a legal requirement or responsibility to share the information.
Your personal information may also need to be shared with third parties to make arrangements for the funding of the services we deliver.
In the event of a reorganisation, merger or sale of Harley Street Ambulance Service or any part of it, we may transfer any and all personal information we collect to the relevant third party.
7) Accuracy of Personal Information
We will always endeavour to keep your personal information accurate, complete and up to date.
It is important that this information is kept up to date and accurate. If any of the information changes, we would need you, relevant relatives or relevant third parties to keep us informed.

For a set period of time, we are legally required to hold certain information about you. Beyond this time, all personal information will be deleted or securely destroyed. We will not keep your personal information for longer than is required or permitted by law.
